Gold Coast Primary Care Opinion Survey

You are invited by Gold Coast Primary Health Network (GCPHN) to take part in the Gold Coast Primary Care Opinion Survey for 2017. 

Your responses will inform GCPHN on how to best support primary care providers on the Gold Coast.

The survey asks GPs, Practice Nurses, Practice Managers and Private Allied Health Professionals within the Gold Coast region about their needs. Four surveys have been developed, each one tailored for the different roles. The more detailed the responses, the more informed we will be to support primary care in the Gold Coast region. 

PLEASE NOTE
If the survey is closed before submission, you will need to start again as unfortunately, there is no save function. Therefore, we suggest if you need to take a break while completing this survey, minimise the web browser with the survey still open in it.

To inform you of the results, a report will be available on our website. GCPHN will use these results to inform what we prioritise in our work with local primary healthcare providers.

Prizes

Terms & Conditions

As an incentive to participate, GCPHN will sponsor a function (lunch or dinner) for two lucky practices/services. Prizes will be randomly selected from the responses submitted before the closing date of Friday 24 February 2017 (terms and conditions below). For each General Practitioner, Practice Nurse, Practice Manager or private Allied Health Professional at your practice who responds to this survey, your practice will receive an extra chance of winning a prize.

In order to go into the draw you will be required to provide your name and practice details at the end of this survey. GCPHN will not share your personally identified information with external third parties. If you would prefer your answers to be anonymous you are not required to provide your details. However, you will not be entered into the draw.

As each General Practice, Allied Health service or Multidisciplinary Medical service differs with the number of staff employed and/or contracted, the amount of funds available for the GCPHN sponsored function will be based on $35 per head up to a maximum per organisation as follows:
 
  • $175 for a small organisation (1-5 people)
  • $525 for a medium sized organisation (6-15 people)
  • $875 for a large organisation (16 people and over)
Prize winners will be drawn randomly from respondents across small, medium and large practices/services until the prize pool is exhausted.
