Better Healthcare Connections Project - BHCC

Gold Coast Primary Health Network is one of nine organisations in Australia delivering the Better Health Care Connections: Aged Care Multidisciplinary Care Coordination and Advisory Services Program. The program supports multidisciplinary care activities to support older Australians with complex health needs who would benefit from higher levels of coordinated care and treatment.

As part of the program Gold Coast Primary Health Network has established an aged care multidisciplinary care coordination and advisory service and funded a pilot that will deliver General Practitioner consultations to aged care recipients in residential aged care facilities by video consultation.
The following activities are being undertaken as part of the project:
  • Trial of an Aged Care Multidisciplinary Care Coordination and Advisory Service.
  • Engagement of participating residential aged care facilities to support GP consultations for recipients of aged care services via video consultation.
  • Implementation of a pilot program of GP consultations for clients of participating facilities.
  • Administration of claims processing and payments to GPs who provide video consultations to clients of participating facilities.

The role of the Aged Care Coordinator includes:
  • Develop coordinated care protocols for aged care recipients with complex needs.
  • Facilitate GP-led multidisciplinary care for recipients of aged care services.
  • Liaise with local/regional health services – primary, specialist, allied health and acute.
  • Improve health outcomes for aged care recipients, people with complex needs within the Medicare Local region.
  • Consult with GPs, practices, facility staff, residents/families and members of a diverse multidisciplinary team.
  • Assist partners to navigate the aged care service system, through partnerships with multidisciplinary service providers.
  • Build a diverse and strong network ensuring access to multidisciplinary teams to support care excellence for residents. 

As part of the project, participating residential aged care facilities will collect and collate data, provide access to the required equipment and initiate requests for telehealth consultations with GPs as required by the resident. The facilities will also allow access by the multidisciplinary teams to consult with the resident as requested by the GP.

The role of the GP will be to utilise the option of a teleconsult with residents within the partnering facilities, and to refer residents to multidisciplinary teams within the community which previously may not have been available. One of the many benefits to residents and GPs is that residents can now attend consultations as though they are living in the community by calling the practice and arranging an appointment.
